Good Nutrition

The Ballan District Health & Care Dietitan provides individualised and practical dietary advice to help individuals of all ages manage their health condition in order to achieve an optimum quality of life. The dietitian can also provide general dietary advice in relation to weight loss (or gain) and to prevent long term health conditions.

Fees

1. Private patients $75 for an initial 30 minute appointment and $55 for a follow up for non concession card holders. HiCAPS is available at reception for processing your claim through your private health fund minimising out pocket expenses on the day of treatment (dependent on your level of cover with your insurer)

2. Enhanced Primary Care (EPC) – No charge if referred by a community health nurse (or GP) under a health management plan

3. Currently, all other non-private patients are charged $9.30, but a different fee schedule will apply from 31 July 2017 (see the Allied Healthcare Service Update below).

Allied Healthcare Service Update

From July 1 2017, a number of Ballan District Health & Care’s allied healthcare services are being funded to deliver a chronic condition model of care. This model of care focuses on community members living with:

  • Diabetes
  • Cardiovascular disease
  • Chronic obstructive airways pulmonary disease
  • Asthma, or
  • Muscular-skeletal conditions.

The purpose of this Commonwealth Government model is to empower community members with high quality evidence-based information, allied healthcare support and timely referrals to strengthen self-management.

The model is likely to benefit community members living with the identified conditions and the funding allows Ballan District Health & Care to continue to employ allied healthcare providers.

It is a condition of this Government funding that a fee is charged. New fees will therefore apply to some allied healthcare services from 31 July 2017.

Fees are based on self-reported income brackets and are capped. There is also a provision to waive fees for community members experiencing financial hardship.

If you are an existing allied healthcare patient or would like more information about how these changes may affect you, please contact your provider in the first instance about the 31 July 2017 fee changes and the changed model of care. Physiotherapy fees for private patients remain as charged by the physiotherapists.

We understand that changes to services and changes in fee arrangements can be challenging and we want to assure you that these changes are a funding body requirement.

Financial Hardship: If you are experiencing financial hardship you may be eligible for a fee waiver or a capped monthly fee. This is at the discretion of the service provider.
