Ballan District Health and Care offer highly qualified community health nurses who work closely with our General Practitioners to conduct a multi-disciplinary health assessment to assist with the management of chronic or lifestyle diseases such as Cardiovascular Disease, Asthma and Type 2 Diabetes.

Part of this service is to also provide 4 year old Healthy Kids Checks, 45-49yr old Health Checks and 75yr+ health assessments, where upon completion of the assessment an individualised goal based health improvement plan will be developed in conjunction with you to help you better understand and improve your health condition.

By having one of these health assessments and plans completed, you may be eligible to access 5 free appointments with our allied health providers to help you manage your condition. Our community health nurse will also stay in regular touch with you after completing your assessment and plan, and will assist you to make appointments to see other health providers or find any other information that you may require.


No cost.


No eligibility criteria. You can refer yourself or a family member to this service.

When & Where

Appointments are available Monday to Friday 8:30am to 4.30pm from our Community Health Centre at 164 Inglis Street, Ballan. Please note that an appointment with one of General Practitioners is also required when you book to see our community health nurse. Our community health nurse will help you arrange this appointment for you.
